Below you will find resources to assist you with enrolling your child at our Academy.

Complete the Waitlist Form

Completing the Waitlist form serves as a notice to JBA that you are interested in enrolling at our Academy. Once completed, you will receive a follow-up email containing our official Enrollment Packet.

Complete Enrollment Packet

Your next steps email will give you the option to complete a digital form via DocuSign or download a paper form to be dropped off at our office. A for must be completed for each child.

Do you received state assistance?

If so, include our DVN #: 002735544 on your 'State Assistance' document. Please be aware it may take 5-10 business days before your document processes and is reflected in our records.

Register with Procare

Register your child on our system through Procare after completing the Enrollment Packet.

Please Note: There is a $50 Registration fee PER child due in order to complete registration

Schedule Enrollment Appointment & Gather Paperwork Needed for Appointment

B. The following documents are needed for each child seeking enrollment;

BRING ORIGINAL DOCUMENTS, we will make copies:

Original Birth Certificate
Up-to-Date Immunization/Shot Record or Health Department Issued Exemption Card
Health Insurance or Medicaid Card
Up-to-Date Physical (Including HGB, Lead & TB)
Up-to-Date Dental Exam
Parent/Guardian Photo I.D.
Proof of Income for the last 12 months (i.e.: W-2 or 1040, TANF Letter, Snap Card, Child Subsidy Letter, Unemployment Letter of SSI Award Letter)
Remember - Uniforms are Mandatory and cost between $15-$20. Please visit our Uniform Shop for more information.
